Outstanding Cleaning Worker of the Year Application

All nominees will be recognized at the annual Simon Institute Symposium. Each nominee will be presented with an Outstanding Cleaning Worker medal at the awards banquet, and featured in the Outstanding Cleaning Worker of the Year commemorative program. Please note, that there is a fee of $75.00 per nominee (this includes processing fees, the medal award, program printing, etc). You will receive an invoice from the Simon Institute for each cleaning worker that you nominate.
